The privacy of your personal health information is paramount. This privacy policy describes how we protect and handle the personal information that you provide and that we obtain through our web-based service at https://www.HealthInReach.com and related web sites ("HealthInReach").
1. Introduction
HealthInReach is bringing together the latest technology and medical information to empower you to live a healthier life. By consolidating timely, relevant information from experts on wellness, disease, prevention, nutrition, fitness, and laboratory analysis, HealthInReach provides highly personalized health and wellness information taking into account your health profile and your medical data.

Along with other features of the Service, with HealthInReach you can:
– store and manage your health conditions, lab results, and health goals in a central place;

– control your own health and wellness records;

– access online health management tools;

– receive results electronically; and

– view your lab results and understand what they mean for you.
Each person's information is stored and accessed separately in his or her own record.
2. Collection of Your Personal Information
You choose whether to create an account with HealthInReach. The first time you sign in to HealthInReach, you will be asked to create an account. To do so, you must provide personal information, such as your email address, name, and date of birth. To better assess your health and wellness and to tailored content just for you, HealthInReach will request additional information, such as height, weight, and age.  Medical record information and laboratory results you provide or transfer to HealthInReach will also be used as described in this privacy policy.
Your email address will be your username and you will be asked to create a password. We will use the email address you provide to send you an email requesting that you validate your account.
You can review and update your account information and modify, add, or delete any optional information by signing into your HealthInReach account and editing your account profile.
An account allows you to manage your health and wellness records and utilize the tools offered on HealthInReach. Once you have an account, you can create health records for yourself and those you love (to the extent you have right to provide their data). When you create a health record, you become a custodian of that record. You choose what information to put in your records. Examples of the types of information you can store in a record include:
– Basic health information

– Personal information

– Health concerns

– Health goals

– Family history

– Biometric measurements, such as blood glucose and blood pressure

– Lab results
HealthInReach will not transfer your information to anyone without your consent.
When you send email or other communications to us, we may retain those communications in order to process your inquiries, respond to your requests, and improve our services.
3. Account Access and Controls
By default, you are the custodian of any records you create. You can close your account at any time by signing into your HealthInReach account and editing your account profile.
4. How We Use Your Personal Information
We use personal information collected through HealthInReach, including health and wellness information, to enable the services provided through HealthInReach and as described in this privacy statement. We do not use or disclose your information except as described in this privacy statement. In support of these uses, we may use personal information to:
– provide you with important information about HealthInReach, including critical updates and notifications;

– send you email and/or mobile alerts;

– modify your HealthInReach plan to better address your health needs and progress; and

– determine appropriate services relevant to you, your lifestyle and your geographic location.
We may access and/or disclose your personal information if we believe such action is necessary to: (a) comply with the law or legal process served on us; (b) protect and defend our rights or property (including the enforcement of our agreements); or (c) act in urgent circumstances to protect the personal safety and welfare of HealthInReach users or members of the public. Personal information collected on HealthInReach is currently stored and processed in the United States.
We will not sell your data.
5. How We Use De-identified and Aggregated Information and Statistics
We may use de-identified and or aggregated information gathered from HealthInReach and related statistics to improve the services provided through HealthInReach, and for certain research and analysis including clinical medical research. We may also provide this information to our partners for such purposes. De-identified information does not include personally identifiable information such as name, email address, or address, but may include gender, height, weight, postal code, and medical information. Aggregated information is information from multiple users that is not associated with any individual user account.
6. Sharing Records with Testing Providers and Other Providers through HealthInReach
Throughout HealthInReach, you are provided with information about lab testing providers and other service providers (such as Google Health and Microsoft HealthVault) that connect with HealthInReach. You should examine these providers individual privacy policies and terms of service prior to using their services because this privacy policy does not apply to the activities of such providers. We encourage you to contact us if you believe a provider is not protecting the privacy or security of your health and wellness information.

7. Deleting Records
You can delete any health and wellness record by signing in to your HealthInReach account and editing a record's profile.
8. Email and Mobile Alerts
To keep you informed of the latest improvements and alerts relating to your HealthInReach health and wellness plan, HealthInReach may send you emails and or mobile alerts. You have the ability to opt out of receiving emails and/or mobile alerts.
9. Security of Your Personal Information
We are committed to protecting the security of your personal information. We use a variety of security technologies and procedures to help protect your personal information from unauthorized access, use, and disclosure. For example, we store the personal information you provide encrypted on computer servers with limited access that are located in controlled facilities. We restrict access to personal information to HealthInReach employees, contractors, and agents who need to know that information in order to operate, develop, or improve our services. These individuals are bound by confidentiality obligations and may be subject to discipline, including termination and criminal prosecution, if they fail to meet these obligations.
HealthInReach is not a "covered entity" under the Health Insurance Portability and Accountability Act of 1996 and the regulations promulgated thereunder ("HIPAA"). Although HealthInReach is not covered by HIPAA, we are committed to user privacy and have in place strict data security policies and measures, and ensure that users control access to their information. We let users know what information we collect when they use HealthInReach.com, how we use it, and how we keep it safe.

Onward Transfer
We reserve the right to transfer some or all of Your Personal Information if HealthInReach becomes involved in a merger, acquisition, or any form of sale of all of its assets. Doing so will enable us to continue to provide service to you without significant interruption. We will provide notice before Personal Information is transferred and becomes subject to a different privacy policy.
Accessing and Updating Personal Information
You may change the settings of Your HealthInReach account via the "settings" section of such account. You may organize or delete certain information through such settings or, alternately, terminate Your HealthInReach account. Such deletions or terminations will take immediate effect. Residual copies of the deleted information and/or terminated HealthInReach account may take up to 60 days to be deleted from active servers and may remain in perpetuity in backup systems.
When you use our Products and Services, we make good faith efforts to provide you with access to your Personal Information and either to correct this data if it is inaccurate or to delete such data at your request if it is not otherwise required to be retained by law or for legitimate business purposes. Where applicable, we ask individual users to identify themselves and the information requested to be accessed, corrected or removed before processing such requests, and we may decline to process requests that are unreasonably repetitive or systematic, require disproportionate technical effort, jeopardize the privacy of others, or would be extremely impractical (for instance, requests concerning information residing on backup tapes), or for which access is not otherwise required.
